Candidate Connection Survey
Raise voter awareness of who you are and what you stand for by using this questionnaire to submit your profile to Ballotpedia. Your information will appear on Ballotpedia and in our Sample Ballot Lookup Tool.
Overview and instructions

  • Required sections and questions are denoted by an asterisk, and directions are provided at the top of each section.

  • Questions will not necessarily appear on your page in the order you answer them.

  • You will be required to verify your identity after submission. This helps keep both Ballotpedia and your information secure.

  • The questionnaire can take between 30 and 90 minutes to complete, depending on the number of optional questions and length of responses.

 

What to expect

  • Once submitted, your answers are final and your information will be stored securely on Ballotpedia servers. Small grammatical corrections can be made upon request.

  • You will be asked to verify your identity. Verification is free, and Ballotpedia will never request payment to verify.

  • Once you are verified, the information submitted, including points such as religion, birthplace, and profession, will be used to populate your Ballotpedia profile.

  • Answers to open-ended longform questions will appear on your profile and in our Sample Ballot Tool.
